The supply evaluation form is used both to evaluate the performance of the supplier and to document your verification of quality, temperature, labeling, packaging and shelf life of the products you receive.


Upon each provisions supply, the supply evaluation form should be filled out and sent to the ship's caterer.

Keep these documents on file, as monitoring these Critical Control Points is essential and forms part of your HACCP / Self Assessment
